Wellness Calls Can Be Used for Any of the Reasons Below

Customized Response Plan

Should our Wellness Call go unanswered, we will work with you or your loved one to develop a customized response plan. We will determine how many times per day we will try to call. If we don’t get through, you can opt for family members to be notified via email or a call, so they can reach out. This closed loop process helps ensure the best care. If the situation isn’t urgent, let us know, and we will simply make a note of the missed call.

Initial Call

We’ll schedule a thorough phone conversation as soon as we have your information to discuss the care situation in more detail, address any urgent issues, and schedule a face-to-face meeting time

In-Home Care Assessment

In order to create a Cared-4℠ Customized Care Plan that is tailored to your particular needs, we will address all of your questions here as well as our own.

Start of Care

On the first day of care, we will accompany our caregiver to go over the Care Plan and assist with introductions.

Client Visits

After starting care, we will occasionally stop by to check on how things are going and decide whether the Care Plan needs to be adjusted.
